Rhomboid & Interscapular Pain
Rhomboid and interscapular pain presents as intense, focal burning or stabbing knots between the medial border of the shoulder blade and the thoracic spine (T1-T6). It stems from prolonged postural strain and weak lower trapezius fibers.
Common Symptoms
- Persistent sharp knot or hot burning sensation between the spine and shoulder blade
- Pain exacerbated by reaching forward with the arm or typing without forearm support
- Tenderness and palpable muscle spasms along the medial border of the scapula
- Relief when pressing a lacrosse ball or foam roller against the knot
Movements to Avoid
- β Aggressive over-stretching of already over-lengthened rhomboids without strengthening lower traps
- β Lifting heavy weights overhead with rounded shoulders
Rhomboids are often over-lengthened and exhausted from holding up slouched shoulders. Combining gentle rotational stretches (Thread the Needle) with lower trap strengthening (Y-T-W) permanently resolves spasms.
Prescribed Exercise Protocol
Thread-the-Needle Thoracic & Rhomboid Stretch
Deep rotational stretch that releases trigger points between the shoulder blades and mobilizes the mid-thoracic spine.
Side-Lying Thoracic Open Book
Restores upper back rotational range of motion while expanding the anterior chest and costal ribs.
Wall Angels Postural Retractor
Strengthens weak lower and middle trapezius while activating the serratus anterior to correct rounded shoulders.
Levator Scapulae & Upper Trapezius Decompression
Releases the "coat hanger" muscle tension connecting the cervical vertebrae to the superior angle of the shoulder blade.
Prone Y-T-W Scapular Stabilizers
Clinical physical therapy sequence that builds postural endurance in the scapular retractors and rotators.
Seated Eagle Arms Interscapular Opener
Expands the interscapular space and separates the shoulder blades to melt away desk worker knots.
